Compassionate Hearts, LLC was formed by two patients who understand the need for coordinating patients with providers for referral into the New Mexico Department of Health Medical Cannabis Program (DOH MCP).
We offer assistance with completion of other cannabis program related forms such as Patient Personal Production License and Caregiver Applications if needed.
Compassionate Hearts also offers hemp based CBD products for those who are waiting on or do not have their medical cannabis card.
Compassionate Hearts, LLC is HIPAA compliant. We have release of information forms to make the process of obtaining patient medical records easy. Once we have current patient records, we coordinate the patient / provider appointment for referral into the cannabis program.
Our provider also offers Post Traumatic Stress Disorder Evaluations for those who feel they suffer from PTSD. The evaluation requires two appointments. Click on the PTSD Evaluation button below for further information.
The Lynn and Erin Compassionate Use Act defines debilitating medical conditions eligible for enrollment in the program as follows:
New patients are required to have current medical records that support the qualifying condition they are filing under. Patients with severe chronic pain must have debilitating pain lasting more than three months and current medical record from within the previous 6 - 9 months.
Renewing patients will need to bring a previous copy of their cannabis application or medical records regarding the approved condition they previously applied under.

The Medical Cannabis Program (MCP) was created under the Lynn and Erin Compassionate Use Act. The purpose of this Act is to allow the beneficial use of medical cannabis in a regulated system for alleviating symptoms caused by debilitating medical conditions and their medical treatments. The New Mexico Department of Health administers the MCP in accordance with the Act. A Primary Caregiver (PC) may be designated by the Qualified Patient (QP) to take responsibility for managing the well-being of the QP in the use of medical cannabis.
The Program has 30 days to approve a completed application form from the date it is received in their office. The approved patient will receive their medical cannabis ID card along with a list of approved dispensaries and patient regulations.
The patient's license is valid for one year, and it is the patient’s responsibility to submit an application for renewal at least 30 days before their card's expiration. The application for renewal may be submitted as early as 90 days prior to expiration, and the Program strongly encourages patients to submit applications at least 60 days prior to their card expiring.
A qualified patient is a resident of the State of New Mexico and is required to submit a completed patient application form applying under one of the approved QUALIFYING CONDITIONS.
The completed application consists of:
Patients who wish to grow medical cannabis for themselves may complete the Medical Cannabis Personal Production Application.
A qualified patient who holds a valid personal production license is authorized to possess no more than four mature female plants and a combined total of 12 seedlings and male plants, and may possess no more than an adequate supply of usable cannabis, as specified in department rule. The personal production license expires when the patient's cannabis registry ID expires.
The primary caregiver of a qualified patient who holds a personal production license may assist the qualified patient to produce medical cannabis at the designated licensed location that is identified on the personal production license; the primary caregiver may not independently produce medical cannabis.
"Primary caregiver” means a resident of New Mexico who is at least 18 years of age and who has been designated by the qualified patient or their representative and the patient’s practitioner as being necessary to take responsibility for managing the well-being of a qualified patient with respect to the medical use of cannabis pursuant to the provisions of the Lynn and Erin Compassionate Use Act, Section 26-2B-1 et seq., NMSA 1978.
The caregiver is required to provide a completed NATIONWIDE background check, done by any national criminal background check company, for all 50 States based on social security number and date of birth.
Minors (children under age 18) are required to have a Primary Caregiver.
